Mental Health - Lead Intensive Case Manager

About the Role

Keystone Human Services is seeking a Lead Intensive Case Manager to provide individualized, person-directed support in the area of mental health. Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer.

Responsibilities

• Assist adults with mental illness gain access to needed resources such as medical, social, educational, and other services
• Mentor other Intensive Case Managers on the team
• Assist in case note reviews and approvals

Requirements

• Minimum Educational Qualifications: (one of the following)
• Master’s degree in social work, psychology, rehabilitation, activity therapies, counseling, or education and three (3) years of direct care experience in mental health
• Bachelor’s degree in nursing and three (3) years of direct care experience in mental health
• Bachelor’s degree in social work, sociology, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, or education; or be a registered nurse; and five (5) years of direct care experience in mental health including two (2) years of supervisory experience

Additional Minimum Requirements

• Effective communication and proficient computer skills
• Ability to resolve complex issues and respond effectively in crisis situations
• Valid driver’s license with daily access to a privately maintained and insured vehicle
