Position Summary
The Systems Integration Facilitator will provide interagency team meeting (ITM) facilitation and leadership that is comprehensive, highly skilled, objective and neutral, trauma-informed, and person-centered for assigned children, adolescents, and transition age youth with complex needs within the Bucks County Behavioral Health system.
Responsibilities
- Engage families, youth, and system partners in exploring strengths, needs and barriers that impact the youth.
- Receive, review and determine program eligibility for new referrals.
- Facilitate an ITM planning process that is strengths based, objective, trauma-informed, and person centered.
- Attend monthly meetings with county stakeholders to report on activities and system barriers.
- Create monthly invoice/reports for program.
- Aid in the preparation of the annual data report.
- Perform additional duties or tasks, as assigned, or requested.
Required Qualifications
- Master's degree in human services or related field.
- One or more years of experience with team dynamics and systems work, e.g. family systems.
- One or more years of experience working with cross systems education, e.g. D&A and the Act 53 process.
- One or more years of experience working in case management.
Preferred Qualifications
- Excellent leadership, partnering and customer service skills.
- Expertise with conflict resolution and management.
- Knowledge of Bucks County children’s serving systems.
- Confidence in public speaking.
Work Environment
A hybrid work environment both in a standard office environment, and remotely from home, as well as community settings to include client home, schools, court, doctor’s offices and hospitals.
Compensation and Benefits
- Hourly rate of $21.50-$22.50 an hour.
