Mental Health Coordinator

Job Summary

The Manhattan Adult Day Care Center is seeking a full-time Mental Health Coordinator to oversee and coordinate the Mental Health Program. The position reports to the Administrative Manager and works Monday through Friday from 9:30 AM to 5:30 PM.

Essential Duties And Responsibilities

• Coordinate all aspects of the Mental Health Program, including intake evaluations, attendance monitoring, problem resolution, and staff oversight.
• Provide direct supervision and guidance to program staff, coordinate case management and care planning activities, and ensure compliance with regulatory and organizational standards.
• Complete initial and ongoing social work and mental health assessments for clients, provide ongoing support, and ensure accurate and compliant documentation.
• Facilitate interdisciplinary care planning meetings, prepare and distribute care plan summaries, and collaborate with external service providers to ensure continuity of care.
• Supervise and develop staff, monitor performance, and report concerns to leadership.
• Maintain client attendance and participation, follow up with providers, review daily activities, identify gaps in program delivery, and develop monthly activity schedules.
• Collaborate with transportation staff to ensure safe and efficient transportation services, and respond to participant emergencies as a team leader.
• Develop and maintain collaborative relationships with community agencies and promote coordination of care.

Skills & Requirements

• Specialized Skills and Competencies: Knowledge of behavioral health, care coordination, case management, and social service systems; strong supervisory, organizational, and crisis intervention abilities; ability to establish productive relationships with diverse stakeholders.
• Experience Requirements: Minimum of 5 years of experience in adult behavioral health or mental health settings, community health programs, and supervisory roles in social work or behavioral health services.
• Education and Licensure Requirements: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Mental Health Counselor (LMHC); additional behavioral health certifications may be preferred.

Frequent interaction with clients experiencing mental health and behavioral health challenges; ability to respond to emergencies and crises; collaborative interdisciplinary healthcare environment; occasional travel to community meetings, client-related appointments, or agency-sponsored activities as needed.
