About the role
The primary clinician provides transitional and comprehensive mental health services, including case management, group and individual therapy, family psychotherapy, and crisis intervention, to a caseload of adults who are high risk for psychiatric hospitalization.
Responsibilities
- Maintain a clinical caseload and provide case management, individual patient/family meetings, psychosocial assessments, and treatment planning with the team.
- Facilitate psychotherapy skills groups, including psychoeducational, supportive, dialectical behavior therapy, cognitive behavioral therapy, and mindfulness groups with diagnostically diverse adult patients; crisis triage/management.
- Complete psychosocial/functional assessments, mental status examinations, comprehensive treatment plan development, and discharge planning.
- Coordinate treatment among various agencies and advocate for clients to link with needed resources.
- Work with the treatment team and inpatient/outpatient units to facilitate referrals and services.
- Attend staff meetings and treatment planning meetings.
Qualifications
- A Master's degree from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; Or an equivalent combination of education and experience;
- Knowledges and abilities essential to the successful performance of the duties assigned to the position.
- Minimum three-years direct patient care experience in a mental health setting, group, and crisis management skills.
- Demonstrated ability to provide transitional, comprehensive mental health services, including psychotherapy, crisis intervention, outreach, and case management to a caseload of severely mentally ill adults who are at high risk for psychiatric hospitalization.
- Demonstrated ability to complete psychosocial/functional assessments, mental status examinations, comprehensive treatment development, and discharge planning.
- Excellent documentation and clinical skills.
- Ability to prioritize; work independently, excellent follow-up, work well under pressure and deadlines.
- Demonstrated excellent attendance and reliability.
- Demonstrated ability to assess suicide and violence risk.
- Demonstrated ability to work harmoniously with colleagues as a member of the treatment team.
- Excellent communication and interpersonal skills.
