Mental Health Case Manager - Republic

About the role

The Mental Health Case Manager provides direct support and case management services to adults experiencing behavioral health conditions. In this role, you will help clients improve stability and functioning by assessing needs, coordinating resources, teaching daily living skills, and supporting individuals through crises.

Responsibilities

• Connect clients to housing, income, benefits, medical and behavioral healthcare, and other essential services.
• Work as part of an interdisciplinary team under the direction of a Mental Health Professional, providing mobile crisis response, follow-up and stabilization services, and assisting clients in developing and achieving individualized service plans.
• Maintain strong collaboration with community partners including crisis responders, hospitals, law enforcement, social service agencies, and housing providers to ensure coordinated, client-centered care.
• Maintain timely and accurate documentation, support caregivers, participate in staff meetings, attend trainings, and contribute to program decision-making.

Requirements

• Bachelor's degree in psychology, social work or related human services field required.
• Able to register as an Agency Affiliated Counselor through Washington Department of Health upon hire.
• Must have an understanding of harm reduction strategies, along with a demonstrated passion for serving individuals experiencing behavioral health disorders.

Pay

• $29.70 - $39.87/hour.
