Mental Health Care Coordinator (Internal title: Clinical Care Coordinator)

About the role

Catholic Community Services, Family Behavioral Health is seeking a Mental Health Care Coordinator (Internal title: Clinical Care Coordinator) to join our innovative team. Our teams provide individualized, creative, and flexible services, infused with evidence-based and evidence-informed strategies.

Responsibilities

• Assess for immediate safety and stabilization needs, develop treatment plans, and create safety and crisis plans.
• Engage families with compassion, support, respect, and enthusiasm, and communicate effectively.
• Provide and/or arrange crisis response and stabilization services, and communicate safety/crisis plans to family and others.
• Respond promptly and collaboratively to crises, preparing families for next steps such as team development and planning processes.
• Customize helping approaches to fit family uniqueness, personality, culture, and interests, brainstorming and negotiating strategies that build on strengths.
• Consult with supervisors, mental health specialists, and others as needed, incorporating recommendations.
• Maintain productivity and documentation standards, using collaborative problem-solving strategies for complete, accurate, and strength-based records.
• Facilitate individual and family therapy, utilize parent/family expertise, and modify intervention techniques as needed.
• Coordinate foster care placements as needed, ensuring necessary paperwork is completed and notifications are made.
• Facilitate implementation of the plan and reevaluate, modify, and redesign the plan based on new information.
• Inspire confidence in the child, family, and team members about their strengths and ability to transition successfully.
• Create and practice an aftercare plan for ongoing services and supports.
• Participate in an on-call rotation with other clinicians and coordinators.

Qualifications

• Bachelor’s degree in social work, psychology, behavioral sciences, or equivalent.
• Experience serving children and/or families (e.g., mental health, social services, education), preferred.
• Ability to drive to family homes and within the community, with reliable transportation, valid driver’s license, and current auto insurance with an acceptable driving record per CCS policy.
• Ability to visually assess safety within the community and family homes.
• Preference for a flexible schedule rather than a traditional, fixed, 8-5, Monday through Friday, facility-based position.
• Ability to participate in on-call rotation responsibilities.
• Pass required background checks prior to employment.

• Demonstrate necessary attitudes, knowledge, and skills to deliver culturally competent services and work effectively in multi-cultural situations.

Benefits

• Starting base pay range: $34.39 - $38.25 per hour.
• Availability pay for after-hours coverage: $75.00 per pay period (2 times per month).
