Responsibilities

• Develop and implement individualized wellness plans for residents with memory impairment, incorporating therapeutic activities, exercises, and social events
• Collaborate with the interdisciplinary team to ensure comprehensive care and support for residents
• Conduct assessments and evaluations to identify resident needs and monitor progress
• Design and lead group activities, such as art therapy, music therapy, and exercise classes
• Provide education and support to residents, families, and caregivers on memory care strategies and techniques
• Maintain accurate records and reports on resident progress and care plans

Requirements

• Current LPN license in the state of Florida
• Prior experience in a long-term care setting, preferably in memory care
• Strong understanding of memory impairment and its effects on daily life
• Excellent communication and interpersonal skills
• Able to work effectively in a fast-paced environment
