About the Role

As a Clare Bridge Program Assistant, you’ll support the Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered, dementia-friendly programs tailored to each resident’s needs. This role aligns with Brookdale’s Resident and Family Engagement philosophy, ensuring meaningful and purposeful dementia care programs that enhance residents' quality of life. Opportunities for career advancement include roles such as Clare Bridge Program Managers, Resident Programs Coordinators, and Business Office Managers.

Responsibilities

  • Supports the implementation of person-centered programs to meet individual resident needs and interests.
  • Assists in leading and engaging residents in the 7 domains of well-being programs (purposeful, intellectual, social, spiritual, emotional, and physical) in a dementia-friendly structured environment, including sensory-focused small groups for residents who cannot participate in large group settings.
  • Assists the Program Manager or Coordinator in reviewing individual resident service plans and completes program documentation as required by Brookdale standards or state regulations.
  • Ensures dementia care programs comply with state, federal, and other regulations while meeting Brookdale’s mission, policies, procedures, and standards.
  • Provides guidance and resources to associates on the importance of programming/engagement and other areas regarding residents' quality of life.
  • Coordinates and participates in special events at the community, including family socials, marketing/sales events, and open houses.
  • Communicates positively with residents, families, and community professionals about the Brookdale dementia care program and its benefits, including sharing residents’ personal needs and successes with families.
  • Assists the Manager or Coordinator with meeting department quality improvement initiatives, including the Brookdale Excellence Standards Tool (BEST).
  • Supports plans and scheduled programming events outside the community, which may include driving a community motor vehicle.
  • Assists with the volunteer program, including training and supervising volunteers.
  • Performs personal care, housekeeping, and dietary responsibilities as needed to maintain service quality in accordance with Clare Bridge Program standards.

Requirements

  • High School Diploma or equivalent required; Associate Degree preferred.
  • Minimum one year of previous experience with older adults and persons with dementia preferred, or an equivalent combination of education and experience.
  • Valid driver’s license and a current 3-year driving abstract with no more than one moving violation (required as a condition of employment).

Skills

  • Basic technical skills to complete repetitive and well-defined duties.
  • Basic knowledge of the organization to answer routine questions.
  • Proficiency with PCs and word processing software, preferably in a Microsoft Windows environment.
  • Effective oral and written communication skills.

Physical Demands and Working Conditions

  • Standing, walking, sitting, and use of hands/fingers to handle or feel.
  • Reaching with hands and arms, climbing or balancing, stooping, kneeling, crouching, or crawling.
  • Talking or hearing, and ability to lift up to 50 pounds.
  • Vision requirements for interaction with co-workers, residents, or vendors.
  • Occasional weekend, evening, or night work to ensure shift coverage.
  • On-call availability as needed.
  • Possible exposure to communicable diseases, infections, latex, blood-borne pathogens, and various drugs, chemicals, or biological hazards.
  • Occasional travel required.

Temporary associates are not benefits-eligible but may participate in the company’s 401(k) program. Pay

Base pay is determined by the applicant’s skills and experience.
