Medicine Remote Care Coordinator

About the role

Provides in-office patient outreach, care coordination, documentation, and EPIC management. Schedules follow-up appointments, reviews discharge instructions with the patient, and serves as the main point of contact for post-procedure follow-up questions.

Responsibilities

  • Conducts patient outreach calls for various programs: Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM).
  • Captures and documents patient interactions and care activities in EPIC.
  • Coordinates and schedules patient follow-up appointments.
  • Reviews and updates patient medications, labs, and patient information.
  • Affords support and documentation for Remote Patient Monitoring (RPM).
  • Maintains provider in-baskets and assists with patient communication.

Requirements

  • High school diploma or equivalent required.
  • Graduation from an accredited training program (school) or 5+ years of supervised work experience in a medical setting.
  • Medical Assistant certification (such as NAHP, NCCT, AAMA, NHA, AMT, etc.) is required.
  • Prior outpatient clinic or physician practice experience is required.
  • EPIC experience is required.
  • Experience with CCM, PCM, or Remote Patient Monitoring (RPM) is preferred but not required.
  • Knowledge of software used by department.
  • Knowledge of general customer service practices.
  • Ability to deal effectively and diplomatically with team members and public.
  • Ability to work in a team environment.
  • Must be self-motivated and self-starter.
  • Attention to details.
  • Self-starter and able to work independently in a dynamic and rapidly changing environment.

Qualifications

  • Alabama, Delaware, Florida, Georgia, Pennsylvania, Tennessee, Texas, & Maryland residents are considered.

The referenced base salary range represents the low and high end of University of Maryland’s Faculty Physician’s Inc. salary range for this position. Exact salary will ultimately depend on multiple factors, including the successful candidate's geographic location, skills, work experience, market conditions, internal equity, responsibility factor, and span of control, education/training, and other qualifications.
