About the Role

We are seeking a dedicated Life Services Coordinator - Caregiver to join our Maggie's Place Memory Care team. The Life Services Coordinator is responsible for cultivating an environment that maximizes each resident’s independence and delivering assistance with personal care, social and recreational activities, meals, and other tasks to facilitate the continued well-being of residents.

Responsibilities

• Provides services to residents as assigned and as indicated on Resident Service Plans, which may include grooming and dressing guidance, assistance with bathing.
• Reports any significant resident changes to the nurse.
• Monitors for changes in resident needs, preferences, and/or health status, and reports/documents any changes according to established procedures.
• Reviews the Staff 24-Hour Communication log and corresponding Resident Service Notes on a daily basis for information about changes in resident service needs and/or health conditions.
• Prepares and serves meals and snacks, followed by clean-up.
• Monitors food expiration dates and maintains kitchen cleanliness.
• Makes beds and changes linens.
• Provides laundry service.
• Performs housekeeping duties, including dusting, vacuuming, cleaning bathrooms, and common spaces.
• Takes out garbage.
• Maintains activity calendar and engages residents in activities such as spiritual, educational, games, music, and physical activities.
• Provides companionship.
• Works effectively with minimal supervision and in a team setting.
• Follows safety policies in performing care, ensuring resident safety.
• Follows and upholds policies and procedures.
• Attends and participates in facility meetings and in-service training programs as assigned.
• Follows infection control guidelines.
• Observes residents’ rights, including privacy and dignity.

Schedule

Part-time, Days: 2:00 PM – 10:30 PM
