Medication History Pharmacy Technician II

About the role

We are seeking a certified pharmacy technician who supports patient care teams by obtaining and documenting accurate medication lists for hospital admissions and surgical care.

Responsibilities

• Conduct thorough interviews with patients and their families
• Contact outpatient pharmacies, long-term care facilities, and other healthcare providers to verify medication information
• Enter good faith effort medication list in the electronic medical record, ensuring accuracy and completeness
• Support treatment planning, medication orders during hospitalization, and post-discharge care

Requirements

• 3-5 years of recent retail experience
• Good organizational and multitasking skills
• Comfortable with patient interactions and communication with providers and pharmacists
• 2+ years relevant experience preferred
• Criminal background check clear of drug-related offenses
• Strong mathematical calculation skills
• Working knowledge and skills as a pharmacy technician in assigned areas
• Intermediate proficiency in typing and using Microsoft Office applications

Qualifications

• High School Diploma or GED
• Indiana Pharmacy Technician License OR intern/extern license if enrolled in an accredited college of pharmacy
• Pharmacy Technician National Certification required unless enrolled in an accredited college of pharmacy

• Intermediate proficiency in typing and using Microsoft Office applications

Pay

Compensation is commensurate with experience.
