Medication Collection Tech II
About the role
The Medication Collection Technician (MCT) II is an advanced level position responsible for comprehensive collection, documentation, and validation of patients’ home medication histories across the continuum of care. In addition to core responsibilities, the MCT II serves as a subject matter expert, supporting quality improvement initiatives, mentoring staff, optimizing workflows, and collaborating across disciplines to enhance medication safety and clinical decision-making.
Responsibilities
- Collects, documents, and validates patients' home medication histories across the continuum of care.
- Serves as a subject matter expert, supporting quality improvement initiatives.
- Mentors staff, optimizing workflows, and collaborating across disciplines to enhance medication safety and clinical decision-making.
- Leads process improvements and contributes to organizational goals related to medication reconciliation accuracy, patient safety, and regulatory compliance.
- Thoroughly knowledgeable of pharmaceutical products, generic names, routes of administration, dosage forms, medical abbreviations, medical terminology, and disease states.
- Working knowledge of medical abbreviations, medical terminology, and disease states.
- Good level of judgment and ability to work independently.
- Typing skills (30 words per minute).
- Working knowledge of new computer programs to quickly and efficiently navigate clinical information systems and pharmacy-related databases.
- Excellent organizational skills required due to the fast-paced and dynamic workflow for this role.
- Pays close attention to detail.
- Demonstrates basic mathematical skills.
- Presents and operates in a professional manner which commands respect, reflects a high level of competency, and encourages congenitally.
Requirements
- Current California Pharmacy Technician license required.
- Pharmacy Technician Certification (CPHT) preferred.
- Associates degree or a technical certification in a healthcare related program or equivalent knowledge and experience preferred.
- Three (3) or more years of pharmacy technician experience required.
- One (1) year of medication collection experience required.
- Demonstrated advanced knowledge of medication reconciliation processes and Meditech system.
Qualifications
