How You’ll Make an Impact

Aid the healthcare team in understanding the social and emotional aspects of clients' health issues.

Conduct clinical social evaluations and develop appropriate intervention plans based on the evaluations.

Identify community resources to assist clients and their families with practical and environmental needs.

Advocate for clients and their families to navigate community systems and ensure their health, safety, well-being, and independent living.

Educate patients, family members, or other patient representatives about available community resources to support their health and independence.

Accurately document observations, interventions, and evaluations of patient care using a touchpad tablet.

Qualifications

• A minimum of a master's degree in social work, with at least two (2) years of community-based experience
• Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree
• Home Health Care experience a plus
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
• Ability to travel to cases, as assigned

• We’re Hiring: Honolulu area - ideal MSW will be able to make visits in a larger geography which they can self-schedule
• Type: Per Diem
• Base Pay, depending upon qualifications and subject to negotiation: $80-$90 per point
