Medical Social Worker (PRN)

About the role

We are seeking a compassionate and dedicated Licensed Social Worker to join our home health care team, providing vital mental health services to individuals in their homes.

Responsibilities

• Conduct psychosocial assessments for patients and families, identifying needs, barriers, and resources
• Develop and implement care plans addressing emotional, social, and practical needs
• Provide counseling, crisis intervention, and patient/family education as appropriate
• Collaborate with clinical, intake, and administrative teams to coordinate services and referrals
• Assist with discharge planning, patient advocacy, and connecting patients to community resources
• Maintain accurate EMR documentation of assessments, interventions, and outcomes
• Monitor patient progress and report relevant information to clinical leadership
• Participate in quality improvement initiatives and case reviews
• Perform other administrative or clinical duties as assigned

Qualifications

• Master's degree in Social Work (MSW) required; LCSW preferred
• Current state licensure as a Social Worker (or eligibility)
• 2–3 years healthcare or home health experience preferred
• Strong assessment, counseling, and communication skills
• Knowledge of community resources, patient advocacy, and care coordination
• Proficiency with Microsoft Office; experience with EMR systems preferred
• Ability to work independently in a fast-paced environment

Schedule

• PRN, Pay-per-visit
• In-person / traveling within a reasonable distance to patients' homes
• Availability during business hours with occasional after-hours/weekend coverage

Pay

$40 - $45 per hour.
