Medical Social Worker - Per Diem

About the Role

The Medical Social Worker, reporting to the Clinical Manager, provides comprehensive social and emotional assessments and develops personalized care plans for home health patients.

Responsibilities

• Evaluate patient’s social, emotional, and medical needs, including relationships and community resources.
• Coordinate with the interdisciplinary care team and supervise the Social Worker Assistant.
• Identify and facilitate access to community resources to support recovery.
• Communicate effectively with patients, families, and healthcare team members.
• Maintain accurate documentation and ensure compliance with policies and regulations.
• Participate in care coordination and discharge planning activities.

Requirements

• Minimum of one year healthcare social work experience; home health or hospice preferred.
• Valid social worker license as per state law.
• CPR certification.
• Valid driver’s license with reliable transportation.
• Knowledge of medications, community resources, and discharge planning.
• Strong organizational, communication, and stress management skills.
• Fluent in English, with flexibility to travel locally.
• TB screening and adherence to driver safety policies.
