Medical Social Worker - Part time - Hospice

Part-time position, 24 hours per week. Field role with a blended caseload of nursing home and home patients, primarily in the greater Nashua area. Occasional weekend days required.
Responsibilities
- Conduct comprehensive psychosocial assessments to identify patient and family needs.
- Develop and implement social work interventions that support patient goals and the overall plan of care.
- Connect patients and families with community resources, financial assistance programs, and supportive services.
- Collaborate closely with physicians, nurses, therapists, and caregivers to coordinate care and discharge planning.
- Facilitate family meetings and provide guidance through complex medical and emotional situations.
- Advocate for patients and assist with benefit applications and other available resources.
- Maintain timely, accurate documentation within the electronic medical record.
- Participate in case conferences, staff meetings, educational programs, and community outreach activities.
- Provide exceptional customer service and patient-centered care.
Requirements
- Bachelor’s degree in Social Work required.
- Master’s degree in Social Work required for Homecare positions in accordance with Medicare regulations.
- Minimum of two years of social work experience in a healthcare setting.
- Experience working with older adults and families.
- Home Health, Hospice, Palliative Care, or End-of-Life Care experience preferred.
- New Hampshire social work licensure preferred.
- Strong assessment, counseling, advocacy, and care coordination skills.
- Valid driver’s license with an acceptable driving record.
