Medical Social Worker - Part Time

ABL Home Health is a provider dedicated to delivering high-quality, compassionate, and patient-centered care in the comfort of clients' homes. The Medical Social Worker (MSW) plays a crucial role in supporting patients and their families through personalized care and community resources.

Responsibilities

• Assess the psychosocial needs of patients and families to develop individualized care plans.
• Provide counseling and emotional support to patients and caregivers.
• Assist in connecting patients and families to community resources, financial assistance, and legal aid.
• Collaborate with nurses, therapists, and physicians to ensure comprehensive care planning.
• Document all services provided in compliance with agency policies and regulatory requirements.

Requirements

• Master's degree in Social Work (MSW)
• 2+ years of MSW experience in a Home Health or similar healthcare setting
• Strong interpersonal, organizational, and counseling skills
• Valid Driver's License and reliable transportation
