Medical Social Worker / Palliative Care

Responsibilities
- Conduct psychosocial assessments (emotional, cultural, financial needs)
- Aid in evaluation and referral for suspected abuse/neglect
- Implement social service interventions; counsel patients/families on illness, treatment, and grief/conflict
- Assist with advance directives and durable power of attorney for health care
- Collaborate with Case Managers and care teams on discharge planning
- Act as liaison between patients, families, care teams, and external agencies; facilitate care planning
- Maintain required documentation in accordance with policies
- Report to assigned manager or director
- May require additional physical demands or respiratory fit testing
- Follow standard precautions due to potential exposure to infections, blood/body fluids, and chemicals
Qualifications
- Master's in Social Work (MSW) from an accredited school
- Hospice and Home Health: Operates an INTEGRIS-owned vehicle or personal vehicle (non-INTEGRIS-owned) and has a current Oklahoma State Driver's License and an acceptable driving record to our insurance carrier
