Medical Social Worker / Palliative Care

Responsibilities

• Conduct psychosocial assessments (emotional, cultural, financial needs)
• Aid in evaluation and referral for suspected abuse/neglect
• Implement social service interventions; counsel patients/families on illness, treatment, and grief/conflict
• Assist with advance directives and durable power of attorney for health care
• Collaborate with Case Managers and care teams on discharge planning
• Act as liaison between patients, families, care teams, and external agencies; facilitate care planning
• Maintain required documentation in accordance with policies
• Report to assigned manager or director
• May require additional physical demands or respiratory fit testing
• Follow standard precautions due to potential exposure to infections, blood/body fluids, and chemicals

Qualifications

• Master's in Social Work (MSW) from an accredited school
• Hospice and Home Health: Operates an INTEGRIS-owned vehicle or personal vehicle (non-INTEGRIS-owned) and has a current Oklahoma State Driver's License and an acceptable driving record to our insurance carrier
