About the role

ABL Hospice is dedicated to providing compassionate, patient-centered care that supports individuals and their families during end-of-life care. Our team of healthcare professionals works collaboratively to ensure dignity, comfort, and emotional support for our patients. We are seeking a Medical Social Worker (MSW) to join our San Francisco team.

Responsibilities

• Assess patients’ and families’ psychosocial needs and develop care plans.
• Provide counseling and emotional support to patients and caregivers.
• Aid families in accessing community resources, financial aid, and legal support.
• Collaborate with the interdisciplinary hospice team to ensure holistic care.
• Maintain accurate and timely documentation in compliance with regulations.

Requirements

• Certified Social Worker - Required
• 2+ years of MSW experience in Home Health or Hospice setting - Required
• Experience in Hospice or palliative care - Required
• Strong interpersonal and counseling skills.
• Valid Driver’s License and reliable transportation.
