Job Description

The Medical Social Work Associate (MSW) at SSMC-Sutter Solano Medical Center provides crisis intervention, information, referrals, and discharge planning for patients and their families, ranging in age from infancy to geriatrics. They serve as an expert and liaison to community resources, coordinate complex discharge planning needs, and make appropriate referrals to support services. They work closely with Clinical Reviewers to ensure high-quality care and outcomes.

Education and Experience

Requires a Master's Degree from a program accredited by the Council on Social Work Education. Two years experience in a Health Care setting and one year acute care hospital experience are required. Previous experience in Discharge Planning is preferred.

Skills and Knowledge

  • Previous experience in Discharge Planning preferred.
  • Strong clinical psychosocial skills with ability to conceptualize clinical situations and demonstrate application of theory appropriate to specific age groups.
  • Knowledge and understanding of individual, child and family development, human behavior and communication, discharge planning and the influence of culture and values in social environments.
  • Ability to plan and implement an effective discharge plan for patients and families.
  • Knowledge of Medicare, MediCal, State and Federal regulations.
  • Knowledge of child, elder, domestic and dependent abuse reporting requirements.
  • Knowledge of Community resources and hospital contracts.
  • Knowledge of Hospital Policy and Procedures.
  • Computer skills, including Microsoft Office applications.
  • Good interpersonal and communications skills.
  • Ability to work under the supervision of the Supervisor of Social Services.
  • Strong clinical assessment and critical thinking skills.
  • Time management skills.

Shift and Schedule

Days Schedule: Per Diem/Casual Shifts

  • Variable Weekend Requirements: As Needed, Occasionally, Rotating Weekends.

The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
