Medical Social Worker II

Schedule: Per diem, weekend hours on an as-needed basis.

About the Role

Provides support and assistance to patients and their families in planning their needs and arranging services accordingly. Coordinates services, facilitates discharge, and ensures access to services. Promotes continuity of care by connecting the Health System, community, and other organizations to deliver integrated care.

Responsibilities

• Demonstrates skill and knowledge of advanced-level social work practice.
• Works independently, exhibiting flexibility in decision-making and prioritizing assignments.
• Develops and maintains collaborative working relationships with team members and community agencies.
• Participates in rounds to facilitate an interdisciplinary approach to hospital care.
• Uses social work theory and practice models to guide patient care plans.
• Proactively resolves problems and recommends continuous quality improvement initiatives.
• Completes mental health assessments as needed.
• Completes assessments for the Birthing Center as needed.
• Develops safety plans with patients as needed.
• Identifies and connects patients with community resources through linkages and referrals.

Requirements

• Master’s in social work from an accredited institution.
• LMSW preferred; MSW with NY state licensure within 12 months accepted.

Qualifications

• Preferred experience in hospital social work.
• Preferred 2 years of experience in the social work field.

Pay

Pay range: $30.00 – $42.00 per hour. Starting pay based on experience.
