Department: Case Coordination • Shift: PRN • Working Hours: Daily as needed • Authorized 8.00 hours bi-weekly
About the role
Demonstrates proficiency, autonomy and accountability in assuring patient care management needs are met from a well-coordinated interdisciplinary and collaborative perspective with case manager and nursing supervisor. Maintains understanding of performance measurements. Takes leadership in Performance Improvement and Customer Service initiatives. Serves as a preceptor.
Must be able to demonstrate the skills and knowledge necessary to provide intervention appropriate to the age, education level, and capabilities of the person served.
Requirements
- Master’s degree in Social Work
- Licensed Master Social Worker (LMSW)
