About the role
BAYADA Home Health Care is seeking a Licensed Medical Social Worker (MSW) for a full-time position performing home health visits from our Pennsauken hospice office. This role serves clients on a per-visit basis in Camden and Burlington counties. Prior home care experience is preferred but not required.
As a Medical Social Worker, you will be an integral member of a multi-disciplinary healthcare team providing skilled nursing and palliative care, enabling clients to receive necessary medical care at home.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Assist the team in understanding the social and emotional factors related to clients’ health problems.
- Provide clinical social evaluations and plan appropriate interventions based on findings.
- Identify and refer clients and families to appropriate community resources to address practical and environmental needs.
- Act as an advocate for clients and families to navigate community systems.
- Participate in developing the total plan of care with the multidisciplinary team.
- Develop and maintain contracts with public and private agencies as resources for clients.
- Educate patients, family members, or representatives on community resources to promote health, safety, well-being, and independent living.
- Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.
Requirements
- Must be licensed by the NJ Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
- Minimum of a Master’s in Social Work with at least 2 years of community-based experience.
- Accreditation by the Council on Social Work Education (demonstrated by school transcript or degree).
- Home care experience is a plus.
- Demonstrated ability to read, write, and communicate effectively in English.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to assigned cases.

Pay
Base pay ranges from $36 to $38 per hour, depending on qualifications and subject to negotiation.
