Medical Social Worker Home Health MSW per Diem

Qualifications

• Masters Degree in Social Work from an accredited program
• One (1) year of Social Work experience in a healthcare setting
• Ability to function independently and demonstrate mature judgment

Duties

• Communicate significant patient information to the health care team and make recommendations
• Collaborate with physicians and other health team members to develop a Plan of Care
• Assess psychological, emotional, and economic factors affecting patient limitations and potential for improvement
• Conduct initial evaluation visits, interim evaluations, and discharge evaluations as required
• Assess patients' needs for long-term care, including home and family situations, and explore alternatives to in-home care
• Counsel patients and caregivers about long-term planning and decision-making
• Document clinical notes each visit, including progress, specific plans, goals, anticipated length of service, referrals made, and follow-up on referrals
• Identify high-risk indicators and provide interventions, reporting concerning situations to Case Managers and appropriate authorities
• Provide brief therapy to facilitate improved coping, adjustment, management, and compliance to medical regimens
• Attend staff meetings, participate in in-services, case conferences, and Quality Assurance Performance Improvement activities as required
• Prepare and submit clerical and progress notes in a timely manner to the office
