Medical Social Worker Home Health MSW NEW per Diem

Qualifications

• Masters Degree in Social Work from an accredited program
• One (1) year of Social Work experience in a health care setting
• Ability to function independently and exercise mature judgment

Duties

• Communicate significant patient information to the health care team and make recommendations
• Collaborate with physicians and other health team members to develop a Plan of Care
• Assess psychological, emotional, and economic factors affecting patient limitations and potential for improvement
• Conduct initial evaluation visits, interim evaluations, and discharge evaluations
• Counsel patients and caregivers about long-term planning and decision-making
• Assess patients' needs for long-term care, including home and family situations, and explore alternative care arrangements
• Identify high-risk indicators and provide interventions as needed
• Report potentially dangerous situations to Case Managers and authorities
• Provide brief therapy to help patients cope, adjust, manage, and comply with medical regimens
• Attend staff meetings, participate in in-services, case conferences, and Quality Assurance Performance Improvement activities
• Prepare and submit clerical and progress notes in a timely manner
