About the Role

A Medical Social Worker (MSW) provides compassionate, patient-centered social services to support clients in maintaining their home living environment.

Responsibilities

• Conduct initial and ongoing social work evaluations.
• Communicate findings and care needs to healthcare teams.
• Implement and modify care plans based on patient progress and needs.
• Counsel patients and families, providing education and support.
• Document case activities accurately and timely.
• Assess psychosocial, financial, and environmental needs.
• Develop and revise care plans.
• Coordinate resources for patient safety and discharge planning.

Requirements

• Master’s or Doctoral Degree in Social Work from an accredited program.
• Current State License as a Social Worker.
• Minimum of 1 year healthcare social work experience.
• Excellent communication skills.
• Ability to travel up to 50% and perform physical tasks including patient transfers.
• Valid driver’s license and reliable transportation.
