About the Role
Provides comprehensive clinical social work services across primary care, inpatient, and home health settings, supporting care teams in managing patients' psychosocial needs.
Responsibilities
- Conduct psychosocial assessments, short-term counseling, advocacy, referrals, and care planning for patients of all ages.
- Collaborate with multidisciplinary teams through care conferences, team meetings, and communication with healthcare providers and families.
- Evaluate and modify care plans based on ongoing assessments to promote patient independence.
- Offer crisis intervention, grief counseling, stress reduction, and coping strategies.
- Assist in resource linkage, care coordination, and ensuring appropriate placements and benefits navigation.
- Coordinate communication among healthcare facilities, community resources, and families.
- Perform referrals, prioritize visits, and manage documentation efficiently.
Requirements
- Master’s degree in Social Work or Behavioral Science.
- Valid state licensure (e.g., LICSW, LMFT, LMHC).
- Minimum one year of social work experience; experience in medical or home health/hospice preferred.
- Strong communication, teamwork, and computer literacy, including electronic medical records.
- Knowledge of chronic/acute illness impact, benefits, and care coordination practices.
- Ability to work independently within a multidisciplinary team and handle complex information.
