Medical Social Worker, Case Management

About the role

The Medical Social Worker provides ongoing case management support to patients and families in the hospital setting, assesses patient needs, and develops discharge care plans. Effective communication with staff, colleagues, patients, and family members is essential.

Responsibilities

• Provide ongoing case management support to the patients.
• Assess patient's needs and develop discharge care plans.
• Communicate effectively with staff, colleagues, patients, and family members.
• Utilize standard Social Work intervention techniques collaborating with patients and families.
• Work as a resource to other staff members.

Requirements

• MSW degree from an accredited School of Social Work required.
• CT State Social Work Licensure LMSW or LCSW preferred.
• Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations and trends.
• Experience with utilization management, home care, community resources, and rehabilitation beneficial.
