Medical Social Worker-BU - Social Services - Sharp Chula Vista Medical Center - Per Diem - Day Shift

Shift: 8:00 AM – 4:30 PM, 8-hour shifts (40 hours per week). Weekend work as needed; no on-call required.

Pay

Hourly pay range: $47.06 – $60.73 – $68.02. Placement within the range is based on years of experience under the collective bargaining agreement between Sharp HealthCare and UNAC Professional.

What You Will Do

• Provide consultation, psychosocial counseling, and support services to Sharp HealthCare patients and families.
• Mobilize patients’ and families’ personal and environmental resources to maximize benefits of medical care and achieve optimal functioning.

Responsibilities

• Assess psychosocial needs:
  • Complete psychosocial assessments.
  • Identify key clinical issues (e.g., grief and loss, adjustment to illness, psychiatric concerns) with attention to developmental milestones and life transitions.
• Communication:
  • Document assessment findings per department guidelines.
  • Consult with team members to develop treatment plans.
  • Maintain ongoing communication with multidisciplinary team members regarding patient status and follow-up.
  • Document ongoing progress with patients/families in the medical record as required by policy and regulatory agencies.
  • Attend rounds in major practice areas.
  • Document consultation, recommendations, interventions, and treatment plans.
  • Identify and communicate needs for patient care conferences.
  • Arrange and participate in patient care conferences for complex/high-risk psychosocial cases.
• Community resources:
  • Utilize and refer to internal and community resources to assist patients and families.
  • Facilitate timely patient discharge.
  • Provide referrals for ongoing community services/support post-hospitalization.
• Consultation and support:
  • Consult and intervene in crisis situations.
  • Assist staff in de-escalating volatile situations.
  • Educate staff on handling Code Green situations.
  • Coordinate and facilitate critical incident debriefings.
  • Develop nursing education to increase staff knowledge.
• Patient advocate:
  • Encourage patient and family participation in decision-making.
  • Report mandatory protective concerns (e.g., elder abuse, child abuse, domestic violence) as required by law.
  • Protect patient privacy and maintain confidentiality.
  • Stay current with regulatory and legal requirements for patient privacy and confidentiality.
  • Identify bio-ethical dilemmas and initiate appropriate responses.
• Patient goals and intervention:
  • Develop and implement treatment goals addressing identified needs.
  • Practice appropriate boundary setting with clients.
  • Demonstrate sensitivity to cultural and ethnic diversity.
  • Apply crisis intervention and problem-solving techniques; serve as a resource to others in these areas.
  • Seek resources for unusual or new situations.
  • Utilize appropriate treatment modalities, including individual, conjoint, and group therapy.
• Plan of care:
  • Evaluate patient/family response to interventions.
  • Revise care provided to patients/families as needed.

Requirements

• Master’s degree in Social Work (required).
• California BBS Licensed Clinical Social Worker (LCSW) (preferred).
• Hospital experience (preferred).
