About the Role
This position will plan, organize, and implement social services for PACE Organization (PO) participants and families including case management and participant advocacy. Responsible for assessment, treatment, teaching, and counseling participants, caregivers, or other appropriate representatives.
May require occasional local travel. Most of the work will be done in the facility, but the position will also require occasional home visits and visits to facilities such as nursing homes and hospitals.
Responsibilities
- Assess participants and coordinate care plans with the Interdisciplinary Team (IDT).
- Conduct intake meetings, provide counseling, and support.
- Act as a liaison with agencies and provide resource referrals.
- Coordinate discharge planning and transitional care.
- Provide individual and family counseling.
- Act as a liaison between the participant and other agencies to facilitate discharge placement planning.
Requirements
- Master’s degree in Social Work field required.
- Two (2) years of experience.
- Current social work license preferred.
- One (1) year working with the frail or elderly population strongly preferred.
- Experience working on a multidisciplinary team in a medical/community-based setting preferred.
- Knowledge of Medicare, Medicaid, and other payment systems.
- Valid driver's license, insurance, and use of a personal vehicle.
- Superior written and verbal communication skills.
- Proficiency with MS Office.
- Ability to manage conflict well, high decision quality, and flexibility to adapt to ongoing change.
