Medical Social Worker

About the Role

The Medical Social Worker applies social work expertise and clinical knowledge to educate patients living with kidney disease, supporting their treatment, health, and wellness to improve quality of life. They conduct psychosocial assessments, participate in care planning, and manage services to help patients cope with chronic kidney disease and dialysis treatments.

Responsibilities

• Support patients’ social and emotional well-being
• Collaborate with a multidisciplinary team
• Assist in psychosocial assessments and care planning
• Provide services to enhance patient coping and adjustment

Requirements

• Master’s Degree in Social Work from an accredited institution
• MSW required; LCSW/ACSW preferred (LCSW in CA)
• 1+ year experience in healthcare or behavioral settings (preferred)
• Licensure as required by state (LMSW, LCSW, ACSW)
