Medical Respite Registered Nurse

About the role

Albuquerque Health Care for the Homeless has been dedicated to serving people experiencing homelessness since 1985. We provide a comprehensive continuum of integrated care through outreach and site-based services, addressing both the health-related causes and consequences of homelessness. Our vision is to live in a world where homelessness is not an issue, focusing on access to quality health care, affordable housing, and a living wage.

Responsibilities

• Work with other disciplines toward integrative approaches to patient care
• Establish a compassionate environment by providing biopsychosocial support to patients
• Manage the care of patients in collaboration with other health professionals and transdisciplinary teams
• Serve as a cross-department clinical resource for AHCH staff
• Collaborate with other staff in treatment planning
• Participate in planning, case review, and quality assurance/quality improvement activities
• Participate in peer review process
• Coordinate continuity of care for AHCH patients hospitalized and through medical respite upon release
• Maintain standards of medical care according to established protocols
• Ensure timely documentation of services in EHR in compliance with AHCH standards

Requirements

• Ability to thrive in a complex healthcare setting
• A strong desire to work in an integrated/cross-disciplinary team model
• Experience documenting services in an EHR
• Knowledge of diagnostic and treatment room procedures
• Experience with health promotion, education, and maintenance
• Knowledge of medications and their effects/side effects on patients
• Excellent communication skills and ability to educate clients on health-related topics
• Able to work with individuals from a wide variety of backgrounds and life experiences in a respectful and culturally competent manner

Qualifications

• Graduate of a School of Nursing program
• Current RN license in New Mexico or eligibility to obtain RN license in New Mexico
• Experience working in a primary or urgent care clinical setting
• Preferred qualifications include a Bachelor’s Degree of Science in Nursing (BSN), 2 years of experience working in a primary or urgent care clinical setting, previous experience with outreach/street medicine, clinical experience with underserved populations, those with SMI/CMI, and substance abuse disorder, previous experience working in a FQHC or community health setting, bilingual Spanish/English, and experience with low cost medical, vision, and dental insurance.

Pay

The pay for this position is competitive and commensurate with experience.

Schedule

The Registered Nurse (RN) position works a flexible schedule, including 24/7 coverage at the Gateway Medical Respite Center, 5400 Gibson SE. This is a co-location role, with most of the time spent at the respite center and some meetings and limit shifts at the clinic downtown. The position may require some nights and weekends as needed. Hours align with the regular campus schedule, 7:30am-4:00pm, Monday-Friday.
