Responsibilities

  • Develop on-boarding classes and simulation lab scenarios for new hires based on identified business and clinical needs, in collaboration with subject matter experts.
  • Provide educational leadership to enhance specialized patient care within established, evidence-based clinical protocols and ensure clinician qualification and competency.
  • Assist with educational needs, problem resolution, and health management across the continuum of care.
  • Collaborate with medical providers, patient care staff, and Home Health management in planning, implementing, and delivering educational curricula.
  • Assist with clinical skills development programs within established clinical and preceptorship models; monitor trends and implement educational strategies to ensure compliance with quality standards.
  • Educate staff on specialized patient care within nursing protocols.
  • Develop patient care plans incorporating evidence-based research and national standards; consult with and serve as a clinical resource for unit management to ensure quality patient care.
  • Educate technical and patient care staff on the use of new equipment, supplies, and instruments; coordinate in-service training and workshops.
  • Ensure proper protocols in the use and maintenance of equipment and supplies.
  • Provide classroom, virtual, self-study, and simulation labs for new hires and staff clinicians as indicated, alongside other educators/facilitators.
  • Evaluate the efficacy of formal and simulation classes to ensure they meet goals and drive results.
  • Determine additional training needs of new hires and existing staff with preceptors, Home Health Leaders, and branch leadership.
  • Collaborate with branch leadership in assessing, implementing, and evaluating clinician, new hire, and preceptor progress toward goals.
  • Facilitate quarterly preceptor forums for continued feedback, mentoring, and preceptor education.
  • Identify concerns with new hires and staff clinicians; communicate these to Home Health Leadership, Lead Preceptor, and branch leadership.
  • Act as a positive role model or preceptor for team members new to home care.
  • Assist with appropriate compliance with state and federal regulatory agency requirements.
  • Participate in clinical process improvement teams and benchmarking activities as assigned.
  • Participate in the development of program changes, including recommendations for obtaining more efficient outcomes; ensure follow-through on practice changes relevant to the role under the direction of the Clinical Support team.
  • Participate in select data collection as requested to track and trend program success; intervene to affect change in program content/delivery.
  • Demonstrate ability to obtain and interpret information in terms of patient care.
  • Function within the Scope of Practice and the Practice Act.

Requirements

  • Associate’s degree in a Healthcare Related field from an accredited or approved school; Bachelor’s degree preferred.
  • 5 years of work-related experience in home health or a similar setting.
  • Current CPR certification.
  • Valid clinical license from the state(s) in which he/she would be practicing.
  • Valid and unrestricted driver’s license.

Skills

  • Thorough knowledge and understanding of clinical protocol, procedures, and standards.
  • Ability to perform all procedures as outlined, with necessary skill and judgment.
  • Ability to provide operational guidance and leadership to technical staff in area of specialty.
  • Knowledge of related regulatory, accreditation, and certification requirements.
  • Ability to assess educational needs and design and develop responsible curricula.
  • Working knowledge of established clinical and preceptorship models in the development of clinical education programs.
  • Advanced clinical skills.
  • Ability to plan, implement, and evaluate individual patient care programs.
  • Highly developed verbal and written communication skills; ability to present effectively to small and large groups.
  • Ability to work effectively at all levels in a collaborative team environment.
  • Ability to prioritize tasks and manage multiple projects.
  • Proficient use of Microsoft Office Suite – Excel, Word, and PowerPoint.
  • Solid ability to learn new technologies; possess the technical aptitude required to understand flow of data through systems as well as system interaction.
