About the Role

Provide outpatient and inpatient care to patients showing a change or decline in mental status, chronic or urgent medical conditions. Work independently within an interdisciplinary team, with responsibilities assigned by the Chief of Medical Services.

Responsibilities

  • Perform medical evaluations, risk factor assessments, and order therapeutic and diagnostic tests.
  • Attend consultations and appointments, and prescribe medications in accordance with clinic policy, formulary, and state regulations.
  • Consult with physicians on cases beyond the scope of an Advanced Practice Nurse.
  • Document all relevant patient data in the electronic medical record and prepare case presentations.
  • Ensure smooth communication with other health professionals for effective clinic service delivery.
  • Provide consultation to program staff to enhance overall quality of care.
  • Diagnose patients with acute and/or chronic illness based on history and physical exams.
  • Act independently with minimal supervision and seek consultation as needed.
  • Transfer patient information to other providers when necessary.
  • Prioritize workload effectively and manage time efficiently.
  • Attend conferences and trainings relevant to clinic practice.
  • Ensure compliance with HIPAA regulations and safety policies.
  • Administer injections with proper technique.
  • Apply evidence-based medicine to clinical decisions and comply with prescriptive monitoring for Schedule II controlled substances.

Requirements

  • Graduate professional nurse and nurse practitioner registration in Massachusetts (unrestricted, current, complete, and active).
  • Certification from the American Academy of Nurse Practitioners (AANP).
  • Strong computer skills, including proficiency with electronic health records.
  • Compassionate, sensitive, and possess strong reasoning and thinking skills.
  • Good oral and written communication skills, with the ability to interact harmoniously with peers.
  • Effective problem-solving skills and ability to handle crisis situations appropriately.
  • Reliable, punctual, and proactive in ensuring a safe work environment.
