About the Role
Provide outpatient and inpatient care to patients showing a change or decline in mental status, chronic or urgent medical conditions. Work independently within an interdisciplinary team, with responsibilities assigned by the Chief of Medical Services.
Responsibilities
- Perform medical evaluations, risk factor assessments, and order therapeutic and diagnostic tests.
- Attend consultations and appointments, and prescribe medications in accordance with clinic policy, formulary, and state regulations.
- Consult with physicians on cases beyond the scope of an Advanced Practice Nurse.
- Document all relevant patient data in the electronic medical record and prepare case presentations.
- Ensure smooth communication with other health professionals for effective clinic service delivery.
- Provide consultation to program staff to enhance overall quality of care.
- Diagnose patients with acute and/or chronic illness based on history and physical exams.
- Act independently with minimal supervision and seek consultation as needed.
- Transfer patient information to other providers when necessary.
- Prioritize workload effectively and manage time efficiently.
- Attend conferences and trainings relevant to clinic practice.
- Ensure compliance with HIPAA regulations and safety policies.
- Administer injections with proper technique.
- Apply evidence-based medicine to clinical decisions and comply with prescriptive monitoring for Schedule II controlled substances.
Requirements
- Graduate professional nurse and nurse practitioner registration in Massachusetts (unrestricted, current, complete, and active).
- Certification from the American Academy of Nurse Practitioners (AANP).
- Strong computer skills, including proficiency with electronic health records.
- Compassionate, sensitive, and possess strong reasoning and thinking skills.
- Good oral and written communication skills, with the ability to interact harmoniously with peers.
- Effective problem-solving skills and ability to handle crisis situations appropriately.
- Reliable, punctual, and proactive in ensuring a safe work environment.
