Location: Spokane, Washington (Community-based / Hybrid)

Responsibilities
- Develop Health Action Plans collaboratively with clients
- Meet with clients monthly in their homes to support goal progression
- Coordinate care with healthcare providers and multidisciplinary team members
- Serve as an advocate and primary point of contact for clients
- Link clients to community resources and support services
- Promote skill-building toward recovery and improved health status
- Support clients in navigating and independently accessing healthcare services
- Maintain accurate and timely documentation in accordance with program standards
Requirements
- Bachelor’s degree in a Human Services, Social Services, Nursing, or related field (Social Work, Psychology, Counseling, etc.)
- Minimum of two years of experience working in nursing, healthcare, behavioral health, or human services (paid or volunteer)
- Valid Washington Driver’s License and active automobile insurance
- Ability to pass state and federal background checks
