About the Role

P3 Health Partners is a population health management group founded and led by fellow physicians who want to shift the industry’s focus from managing illness to cultivating wellness. We are health plan agnostic and work with most plans in our markets. As the conduit between you and the health plan, we aim to make a difference where it counts—with your patients, their families, and the communities where we live. The mission of P3 is to help providers and their patients embrace a new kind of care that gives patients the tools and resources they need to manage their long-term health and wellness.

As a medical assistant at P3 Health Partners, you’ll be a vital member of an interdisciplinary team, transforming lives and empowering primary care physicians to deliver exceptional care to complex medical patients. This full-time role requires a multifaceted approach, encompassing all aspects of care delivery. You will conduct comprehensive geriatric assessments in-office, via telemedicine, or through on-site visits at senior facilities, patient homes, dialysis centers, and other appropriate locations.

Responsibilities

  • Conduct visits in coordination with other P3 team members in a variety of settings.
  • Escalate concerns discovered during patient visits to the designated provider or appropriate team member.
  • Collaborate with the interdisciplinary team to support coordinated, value-based care.
  • Verify patient information using 2 unique identifiers; obtain vital signs, administer geriatric assessment tools, record medical history and medications for review by provider and/or pharmacist.
  • Perform various procedures (e.g., venipuncture, ear lavage, medication administration) and specimen handling.
  • Perform administrative duties as a member of the team.
  • Maintain a safe, secure, and healthy work environment by following standards and procedures and complying with legal regulations.
  • Identify supplies requiring ordering and communicate needs to the appropriate team member.
  • Maintain equipment by following operating instructions, troubleshooting breakdowns, performing preventive maintenance, and calling for repairs.
  • Enhance practice reputation by accepting ownership for new and different requests and exploring opportunities to add value.
  • Communicate opportunities to advance the clinical model.
  • Confidently use provided technology.
  • Ensure patient privacy and adherence to all HIPAA regulations and standards for handling PHI.
  • Participate in quality improvement initiatives, documentation audits, and other activities that support clinical excellence and operational efficiency.
  • Maintain compliance with required trainings, entries into the medical record, meeting attendance, and other requirements.
  • Role model the P3 Health Partners core values.

Requirements

  • Medical Assistant certification required.
  • Experience in home care or house call settings preferred.
  • Experience in geriatric care preferred.
  • Flexible, self-driven, collaborative, and innovative personality.
