Responsibilities
- Assists with direct patient care procedures and related tasks such as:
- Lab tracking and data entry
- Document management
- Obtaining patient histories and vital signs
- Verifying medication lists
- Preparing charts and pre-visit planning
- Schedules patient appointments and follow-up tests taking into consideration medical concerns and patient circumstances as desired.
- Supports clients as they come into the clinic to check vital signs, height, and weight and inputting the data into the electronic medical record.
- Completes screenings using standardized tools to collect information on social determinants of health regarding:
- Transportation
- Housing stability
- Utilities
- Food insecurity
- Domestic violence
- Tobacco use
- Alcohol use
- Provides resources to clients as needed based on information collected on the social determinants of health.
- Aids client/family in scheduling appointments and lab work, relaying details to the client, and assisting with arranging transportation if needed.
- Maintains detailed, accurate, and timely documentation in the medical record.
- Communicates information to the care team in a timely manner.
- Attends supervision, agency, and team staff meetings as scheduled.
- Engages in training on evidence-based practice models specific to their assigned population, including motivational interviewing, implementing learned skills in daily interventions.
- Follows agency policies and standards for client rights and confidentiality.
- Performs other duties as assigned.
Qualifications
- Certified Medical Assistant and/or Certified Clinical Medical Assistant required.
- Basic Life Support Certification required.
- Minimum of 1 year experience, preferably working with individuals with behavioral health conditions.
- Valid driver’s license and use of car to complete major functions of the job.
- Automobile liability insurance coverage for the vehicle being used in accordance with Center requirements.
- CPR training within 30 working days of employment and maintained annually.
Job Knowledge, Skill, and Ability
- Understanding of mental health and substance use diagnoses.
- Understanding of family systems model of care.
- Knowledge of community resources.
- Familiarity with screening/assessment tools and evidence-based practices relevant to the position.
- Oral and written communication skills at a proficient level, ensuring effective and professional communication.
