Job Functions

  • Provides patient care and clinical support in clinics, mobile healthcare units, street outreach settings, and community-based care environments, including travel between care locations as needed.
  • Collaborates with clinical teams, supervisory staff, and community partners to support outreach and engagement efforts for vulnerable patient populations.
  • Participates in daily huddles, interdisciplinary care coordination activities, staff meetings, trainings, and educational opportunities.
  • Registers patients, verifies demographic and insurance information, manages in-basket tasks, responds to patient portal messages, and documents encounters accurately within the electronic health record.
  • Supports patients in accessing healthcare services and community resources by coordinating with care teams and community partners to address barriers to care.
  • Performs duties within scope and Standing Delegation Orders, including administering medications and vaccines, assisting with exams and procedures, collecting specimens, performing EKGs, phlebotomy, and CLIA-waived testing.
  • Maintains patient care areas, supplies, medications, vaccines, and clinical equipment in accordance with infection prevention and organizational standards.
  • Conducts outreach and follow-up activities to support continuity of care, completion of care gaps, referrals, diagnostic testing, scheduling, transportation coordination, and other patient support needs.
  • Communicates respectfully and professionally with patients, families, community partners, and team members while maintaining patient confidentiality.
  • Sustains clinic workflows, including pre-visit planning, rooming, and visit wrap-up activities.
  • Utilizes trauma-informed approaches and de-escalation techniques to support patients exhibiting escalated or disruptive behaviors while maintaining safety.
  • Maintains compliance with all regulatory, accreditation, organizational, HIPAA, and Joint Commission standards.
  • Participates in quality improvement and process improvement initiatives to support patient care and operational effectiveness.

Qualifications

  • High School Diploma or equivalent (higher degree accepted)
  • Completion of accredited Medical Assistant program
  • One year clinical experience
  • CPR/BLS Certification through American Heart Assoc or Red Cross upon hire
  • Texas Driver's License valid upon hire
  • National Medical Assistant Certification upon hire
