Job Overview

The Coordinator, Home Visit Program provides medical services to the Home Visit Program for complex client care cases, ensuring appropriate and professional medical care, focusing primarily on Medicare Annual Wellness Visits, as well as providing care and coordination for home-bound complex care clients.

Minimum Requirements

  • High School graduate or equivalent with a Medical Assistant certification or equivalent required.
  • Minimum of (2) years of back-office experience preferred.
  • Minimum of (2) years of experience in a managed health care environment, preferably IPA, HMO, or health plan preferred.
  • Experience in hospice, palliative care, or high-risk care settings preferred.
  • Bilingual in Spanish and English preferred.
  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation
