About the Role

The Clinical Partner (Medical Assistant) will assist HarmonyCares Primary Care Physicians, Nurse Practitioners, and Clinical Teams in delivering primary health care and patient care management in a residential setting.

Responsibilities

  • Supports mission, vision, and core values by delivering compassionate support, attention, and assistance to patients, families, and team members.
  • Supports patient care through clinical partnership with Nurse Practitioners, Physicians, and Care Management Teams.
  • Coordinates daily travel itinerary and transports clinicians to scheduled appointments.
  • Prepares patients for examination and treatment, including minor procedures.
  • Secures patient consent forms and records patient care documentation accurately and in a timely manner.
  • Coordinates patient care as directed by clinicians, company standards, and policies.
  • Respects patient confidentiality and treats patients with courtesy and respect.
  • Performs phlebotomy (venipuncture only), EKG, or other procedures as requested by the clinician.
  • Labels, stores, and properly packages specimens for lab delivery.
  • Assists in identifying and closing quality gaps.
  • Documents and updates patient care plans, SDoH (Social Determinants of Health), and other assessments within the patient chart or electronic medical record.
  • Maintains compliance with CLIA (Clinical Laboratory Improvement Amendments), OSHA, and HIPAA guidelines.
  • Prepares and maintains supplies and equipment for treatment (supply inventory, medical bag, and car).
  • Ensures company vehicle is clean and follows proper periodic maintenance schedule (where applicable).
  • Attends required meetings and in-services.

Requirements

  • High School graduate or GED equivalent.
  • Graduate of an accredited Medical Assistant, Phlebotomy, EMT, or related medical education/training program, or 1 year of experience as a Medical Assistant or Phlebotomist.
  • Current CPR certification that meets company guidelines.
  • Knowledge of medical terminology.
  • Basic computer literacy, including EMR, iPad, and GPS experience.
  • Strong interpersonal communication and customer service skills.
  • Must maintain a valid driver’s license and good driving record.

Preferred Qualifications

  • Certified Medical Assistant.
  • Six months of experience in patient care, preferably geriatric care.
  • Strong phlebotomy skills.
  • Desire and ability to obtain MA certification within 180 days of hire.

Working Conditions

  • Full range of body motion, including handling/lifting patients.
  • Ability to drive up to 150 miles daily in urban and/or rural settings.
  • Ability to lift/carry items weighing up to 50 lb.
  • Ability to read and understand road maps.
  • Sit or stand for long periods of time.

Pay

Posted pay range: USD $23.00–$24.44 per hour. Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
