Medical Assistant (Clinical Partner)

Responsibilities

• Serves as a Clinical Partner supporting the mission, vision, and core values of HarmonyCares.
• Supports patient care through clinical partnership with Nurse Practitioners, Physicians, and Care Management Teams.
• Coordinates daily travel itinerary and transports clinicians to scheduled appointments.
• Prepares patients for examination and treatment; may include preparing patients for minor procedures.
• Secures patient consent forms, records patient care documentation accurately and in a timely manner.
• Carefully coordinates patient care as directed by Clinicians, company standards, and policies.
• Respects patient confidentiality at all times and treats patients with courtesy and respect.
• Performs phlebotomy (venipuncture only), EKG, or other procedures as requested by Clinician.
• Labels, stores, and properly packages specimens for lab delivery.
• Documents and updates patient care plans, as well as SDoH (Social Determinants of Health) and other assessments within the patient chart or electronic medical record.
• Maintains compliance with CLIA, OSHA, and HIPAA guidelines.
• Prepares and maintains supplies and equipment for treatment (supply inventory, medical bag and car).
• Ensures company vehicle is clean and follows proper maintenance schedule (where applicable).
• Attends required meetings and in-services.

Qualifications

• High School graduate or GED equivalent.
• Graduate of an accredited Medical Assistant, Phlebotomy, EMT, related medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist.
• Current CPR certification that meets Company guidelines.
• Knowledge of medical terminology.
• Basic computer literacy, including EMR, iPad, and GPS experience.
• Strong interpersonal communication and customer service skills.
• Maintain a valid driver’s license and good driving record.
