Responsibilities
- Serves as a Clinical Partner supporting the mission, vision, and core values of HarmonyCares.
- Supports patient care through clinical partnership with Nurse Practitioners, Physicians, and Care Management Teams.
- Coordinates daily travel itinerary and transports clinicians to scheduled appointments.
- Prepares patients for examination and treatment; may include preparing patients for minor procedures.
- Secures patient consent forms, records patient care documentation accurately and in a timely manner.
- Carefully coordinates patient care as directed by Clinicians, company standards, and policies.
- Respects patient confidentiality and treats patients with courtesy and respect.
- Performs phlebotomy (venipuncture only), EKG or other procedures as requested by Clinician.
- Labels, stores, and properly packages specimens for lab delivery.
- Updates patient care plans, SDoH (Social Determinants of Health) assessments, and other assessments within the patient chart or electronic medical record.
- Maintains compliance with CLIA, OSHA, and HIPAA guidelines.
- Prepares and maintains supplies and equipment for treatment.
- Ensures company vehicle is clean and follows a proper maintenance schedule.
- Attends required meetings and in-services.
Qualifications
- High School graduate or GED equivalent.
- Graduate of an accredited Medical Assistant, Phlebotomy, EMT, related medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist.
- Current CPR certification that meets Company guidelines.
- Knowledge of medical terminology.
- Basic computer literacy, including EMR, iPad, and GPS experience.
- Strong interpersonal communication and customer service skills.
- Maintain a valid driver’s license and good driving record.
