Medical Assistant - Chrionic Care Management
About the role
As a member of the Population Medicine team, you will work collaboratively within an interprofessional care team to provide proactive, person-centered support for patients living with chronic medical conditions. Using population health strategies and risk-based outreach to identify patients who may benefit from additional support, the CCM Medical Assistant helps ensure patients receive timely, coordinated, whole-person care that is responsive to their individual goals, preferences, medical complexity, and social needs.
Requirements
- Graduation from accredited Medical Assistant program required
- 3 years related outpatient clinical experience preferred
- WA MAC license required
- BLS required
- Current driver’s license and auto insurance required
- Immunizations required prior to start date (COVID, Hep B (3), Tdap, MMR (2), Varicella (2), TB/PPD, FLU)
- Pre-employment drug screen required
Pay
$20.40 - $27.02 hourly
$1.00 bilingual differential (must pass ALTA)
