Medical Assistant - Care Concierge

About the role

This position involves supporting senior patients through in-person rounds, addressing inquiries, and assisting with scheduling follow-ups. The role emphasizes seamless communication within an interdisciplinary care team and includes other related duties.

Responsibilities

• Manage electronic health records and coordinate documents
• Facilitate telehealth sessions
• Support provider activities such as prior authorizations and review of diagnostic results

Requirements

• High school diploma
• Registered or Certified Medical Assistant
• At least one year of experience in medical or community settings
• Ability to interpret medical records and organize clinical information
• Proficient computer skills and strong time management

Preferred Experience

• Working with chronic care management programs

Travel

On-site at senior living communities multiple days weekly, with possible travel to various facilities. Must be willing to travel and may require a valid driver’s license and auto insurance.
