Medical Assistant

About the role

We are currently hiring for a full-time Medical Assistant for our Transition of Care department to work at our Center City, Philadelphia, PA location. This position will be hybrid and is entitled to a $500 sign-on bonus.

Responsibilities

• Coordinating patient care as they move between healthcare settings, including but not limited to:
  • Post-discharge calls
  • Scheduling follow-up appointments
  • Medication reconciliation
  • Patient education
  • Connecting patients to resources to prevent readmission
• Working closely with providers and care teams on the best plan of action
• Route emergency calls appropriately and escalate when needed
• Perform general administrative support duties
• Other duties as assigned

Requirements

• Certified or Registered Medical Assistant preferred
• Must have at least 1-3 years of experience as a Medical Assistant
• Must have the ability to multitask
• Computer knowledge is a must
• Experience with an EMR system is a plus
• Must be well-spoken, friendly, and outgoing
