About the role
This position involves coordinating medical appointments and necessary communications for individuals living in residential sites, ensuring they receive proper medical care. The role also includes providing direct support and medical supervision.
Responsibilities
- Assist people in receiving needed medical services and documenting pertinent information (e.g. medication administration, transportation).
- Provide medical supervision and review MARs for refusals daily.
- Track injuries and falls, send information to medical providers, and track monthly MAR renewals to the pharmacy.
- Ensure adequate medication availability, communicate with residential site staff, and make recommendations to improve health and wellness.
- Coordinate with CSRN for Pharmacy Reviews, review medical charts, and implement Life Plan and Staff Action Plans.
- Support individuals in accessing community resources, assist with interpersonal relationships, and complete required paperwork.
- Complete household tasks, drive agency vehicles when needed, and promote cooperation among various stakeholders.
- Attend weekly meetings with CSRN and provide direct support as needed.
- Participate in corporate compliance, quality assurance, and improvement activities as required.
- Work independently and as part of a team, adhere to program policies, and demonstrate ethical behavior.
Nonessential Functions
- Provide positive role modeling.
- Participate on agency committees and attend functions.
Requirements
- High School Diploma or equivalent.
- At least one year experience with individuals with developmental disabilities preferred.
- Valid NYS driver's license with a safe driving record.
- Certified Nursing Assistant or Certified Home Health Aide preferred.

Physical & Mental Requirements
- Physically capable of assisting individuals in transfers and preventing falls.
- May need to shovel and remove snow and ice from walkways and driveways.
Working Conditions
- Requires use of office equipment and household appliances.
- May require offsite travel for appointments, events, and outings.
