Responsibilities

• Makes rounds to monitor the health and well-being of residents.
• Provides medication care assistance and administration.
• Communicates with resident support services and families.
• Maintains clinical quality assurance.
• Supports the Wellness Nurse with completing monthly wellness visits and PSP.
• Informs the Wellness Director and/or Executive Director of any changes in medication or service level.
• Absorbs and documents changes in resident care needs.
• Assists with obtaining weights and vital signs monthly for each resident.
• Assists with resident care as needed.

Requirements

• Certified Medication Aide and/or meets state requirements for medication aide.
• One year of experience working with elders or disabled individuals, preferred.
• Demonstrated leadership capabilities.
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
• Available to work weekends, evenings, and flexible hours.
