About the role

The Meadows of Napa Valley is a not-for-profit Life Plan Community located in the heart of Wine Country. It offers residents a range of services including Assisted Living, Memory Care, and Skilled Nursing, along with resort-style amenities.

Responsibilities

  • Provide person-centered, individualized, and clinically competent care with support from the management team.
  • Affirm resident rights and advocate for operations, processes, and decisions that are in residents' best interests.
  • Delegate and assign appropriate tasks to team members.
  • Work closely with team members to ensure accountability, communication, and effective work.
  • Ensure services are appropriate to meet residents' needs in accordance with federal, state, and local regulations.
  • Document findings, notify physicians and responsible parties, and create resident-specific and person-centered Service Plans.
  • Review and maintain 24-hour reports and other pertinent nursing documentation.
  • Assist in the admission of new residents, including assisting them, identifying immediate needs, educating caregivers, and completing the admission assessment and initial admission notes.
  • Promote resident independence to the greatest degree possible.
  • Support in the admission of new residents which includes, but is not limited to, assisting resident, identifying immediate needs, education to caregivers and insuring completion of the admission assessment and initial admission notes.
  • Respond to emergency calls on campus per protocol.
  • Initiate and complete incident reports for any accidents, medication errors, emergency situations, etc.
  • Supervise, assist, and implement infection control measures and prevention to ensure compliance.
  • Perform other duties as assigned.

Requirements

  • Minimum of 1 year's professional nursing experience, especially in long term care.
  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license from accredited nursing program, school, or university.
  • Current CPR certification is required.
  • Demonstrated understanding of functions, organization, policy, regulations, and procedures of a long-term care/skilled nursing facility.
  • Competent in using electronic health records and other clinical software.
