Job Description

• Responsible for documentation verification of all nursing care administered.
• Responsible for resident MDS data collection and documentation, completion of resident assessments, care plan, Rap summaries.
• Completes care plan schedules and meets with families and residents to discuss plans of care.
• Collaborates with the interdisciplinary team to discuss resident plans of care.

Requirements

• Must have Registered Nurse RN license in good standing in Illinois.
• Minimum 3 years relevant experience.
• Previous MDS / Long term care experience preferred.
• Familiar with PDPM requirements.
• Demonstrates caring and compassionate nature.
• Must possess excellent supervisory skills.
• Must possess strong computer skills.
• Candidate must be a team player, have a positive outlook and excellent problem-solving skills.
• History of excellent attendance.
