Lutheran Home is seeking an experienced RN MDS Coordinator to oversee resident assessment and reimbursement processes while partnering with the interdisciplinary team to support quality care. This position offers a consistent weekday schedule with no weekends and no on-call responsibilities. Position Overview As the RN MDS Coordinator, you will manage the resident assessment process from start to finish, ensuring documentation accuracy, regulatory compliance, and timely completion of all required assessments. Your work will help ensure residents receive appropriate care while supporting accurate reimbursement for services provided. Key Responsibilities Lead and manage the MDS/RAI assessment process for assigned residentsReview clinical documentation to ensure assessments accurately represent resident conditions and care needsMonitor assessment schedules and deadlines to maintain compliance with federal and state regulationsCollaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plansSupport Medicare, Medicaid, OBRA, and payer-specific assessment requirementsConduct chart reviews and documentation audits to promote reimbursement accuracyAssist with quality improvement initiatives and survey preparation activitiesServe as a resource for clinical staff regarding MDS and assessment-related processes Qualifications Required Active Registered Nurse (RN) license in the State of IllinoisGraduate of an accredited nursing programMinimum of two years of MDS 3.0 coordination and case management experience in a skilled nursing, rehabilitation, or post-acute care environmentStrong organizational and communication skillsProficiency with Microsoft Office applications Preferred Experience working in long-term care or skilled nursing settingsRAC-CT certification, or ability to obtain certification within six months of hire Compensation & Benefits Annual salary: $87,800 - $99,700, depending on experienceMedical, dental, and vision coverage403(b) retirement planDaily Pay optionPaid time offTuition reimbursement assistanceMental health and wellness resources through WellBeingConfidential chaplain support available 24/7Professional development through our 365 team member growth program Work Schedule Full-time positionMonday through Friday8:00 AM - 4:00 PMNo weekend requirementsNo on-call rotation Apply Today Join a team committed to providing exceptional care and support for older adults.
