MDS Nurse Coordinator - RN
Position Overview
Looking for a role where your clinical expertise directly impacts resident outcomes? Lutheran Home is seeking an experienced RN MDS Coordinator to oversee resident assessment and reimbursement processes while partnering with the interdisciplinary team to support quality care.
Key Responsibilities
Lead and manage the MDS/RAI assessment process for assigned residents
Review clinical documentation to ensure assessments accurately represent resident conditions and care needs
Monitor assessment schedules and deadlines to maintain compliance with federal and state regulations
Collaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plans
Serve as a resource for clinical staff regarding MDS and assessment-related processes
Support Medicare, Medicaid, OBRA, and payer-specific assessment requirements
Conduct chart reviews and documentation audits to promote reimbursement accuracy
Aid in quality improvement initiatives and survey preparation activities
Qualifications
Active Registered Nurse (RN) license in the State of Illinois
Graduate of an accredited nursing program
Minimum of two years of MDS 3.0 coordination and case management experience in a skilled nursing, rehabilitation, or post-acute care environment
Strong organizational and communication skills
Proficiency with Microsoft Office applications
Preferred Experience
Experience working in long-term care or skilled nursing settings
RAC-CT certification, or ability to obtain certification within six months of hire
Compensation & Benefits
Annual salary: $87,800 - $99,700, depending on experience

Full-time position

8:00 AM - 4:00 PM
